One of other thing to note is that you not only need to consider simply changing Dr's. You also have to think about the quality and breadth of services in the area you are moving to. My wife and I have a place on LI in NY where the medical care options are excellent. Top quality care in any area of specialty is ready to come by. However, in upstate NY where my wife has a house, medical care is awful. There's few options and those that are available are largely incompetent for anything beyond the common cold.
